“More older adults are using cannabis. It’s more widely available and is being used for different reasons than in younger folks. Plus, people are living longer. We have to be asking, what are the long-term effects of cannabis use as we continue to age?”
This study used data from the UK Biobank, a massive repository of health information from over 500,000 adults, to analyze patterns of cannabis use, regional brain volumes, and cognitive performance. By focusing on regions of the brain rich in CB1 cannabinoid receptors, the researchers were able to look at nuanced effects rather than broad, overall gray matter changes, a common limitation in prior research.
Cannabis and Brain Volume: Key Regions Affected
Perhaps the most striking finding is that lifetime cannabis use correlated with increased regional brain volumes in areas such as the hippocampus, caudate, putamen, and amygdala, all regions central to memory, learning, emotional regulation, and executive function. For middle-aged and older adults, larger brain volumes may reflect preserved neurological integrity, in contrast to the atrophy and volume loss typically seen with aging. In other words, cannabis use in moderation could be linked to maintaining brain health rather than accelerating cognitive decline.
Importantly, the brain volume gains were accompanied by measurable improvements in cognitive performance. Study participants who reported moderate cannabis use scored better on tests of learning, memory, processing speed, and attention. Even individuals who used cannabis primarily during adolescence displayed slightly larger brain volumes and enhanced cognitive function in adulthood compared with non-users.
However, the effects were not uniform. A notable exception was the posterior cingulate, a brain region involved in memory and emotion, where higher cannabis use was linked to smaller volumes. While this might sound alarming, the relationship between posterior cingulate volume and cognition is complex; some studies suggest smaller volumes in this area may actually correlate with better working memory. The takeaway is clear, cannabis affects the brain in multifaceted ways, and volume changes do not automatically equate to positive or negative outcomes.

Is Moderate Cannabis Use Protective for the Aging Brain?

While there was no consistent pattern showing one sex consistently benefiting more than the other, significant interactions were observed across several brain regions and cognitive measures. “It’s not a case of cannabis being all good or all bad. It depends on how people are using it, what outcomes you’re looking at… and what part of the lifespan are we looking at.”
