Germany’s Medical Cannabis Market Faces a Potency Problem

Germany’s medical cannabis market has undergone a remarkable transformation in a short period. Following regulatory changes that removed medical cannabis from the country’s list of narcotic drugs in April 2024, patients have gained easier access to products through a rapidly expanding marketplace. Prices have fallen, product selection has exploded, and online ordering has made obtaining medical cannabis more convenient than ever.

However, emerging research suggests that Germany’s medical cannabis market is increasingly rewarding the same consumer behaviors typically associated with recreational retail markets, with buyers seeking the lowest prices and the highest THC concentrations. While improved affordability and access may benefit many legitimate patients, researchers argue that this trend raises an important policy question. Should a medical cannabis system allow increasingly potent products to dominate sales when evidence supporting their superior therapeutic value remains limited?

A 2026 study1 published in the International Journal of Drug Policy offers insight as one of the most detailed examinations of how Germany’s medical cannabis market has evolved during its first year under the new framework. Let’s dive into the research and what it means for the future of medical cannabis in Germany.

Germany’s Medical Cannabis Market Expanded Rapidly

Researchers analyzed twelve months of supply and demand data collected from a German online medical cannabis dispensary using web-scraping techniques. The study tracked nearly 1,000 cannabis flower products over the course of the year, recording product availability, THC and CBD concentrations, pricing, stock levels, and changes in demand.

The results illustrate just how quickly the market grew. Available cannabis flower products increased from 266 products in December 2024 to 401 products by November 2025, with a total of 996 unique products appearing during the study period. Patients were presented with an expanding selection of cannabis varieties, offering more choices than ever before.

At the same time, pricing became increasingly competitive. Researchers found that the average price per gram of medical cannabis flower fell by approximately 31% over the course of one year. Even when adjusted for THC content, the cost per 10 milligrams of THC steadily declined throughout the study. For many patients, lower prices improve access by reducing financial barriers to treatment. However, the study found that falling prices also influenced purchasing behavior in important ways.

High-THC Products Dominate the Market

One of the study’s most striking findings was the overwhelming prevalence of high-potency cannabis products. Approximately 85% of all available cannabis flower products contained at least 20% THC. Even more notably, products containing between 20% and 29.9% THC accounted for more than two-thirds of all measured demand. Rather than representing a small segment of the market for patients with specialized needs, high-THC flower has effectively become the dominant product category.

Meanwhile, products with lower THC concentrations represented a much smaller share of both available inventory and consumer purchases, suggesting that Germany’s medical cannabis market increasingly emphasizes potency as a key selling point.

Price and Potency Drive Demand

The researchers examined which factors best predicted consumer demand and found two clear drivers: lower prices and higher THC concentrations. Every one percentage point increase in THC content was associated with a significant increase in product demand. Conversely, higher prices consistently reduced demand.

The relationship between price and purchasing behavior proved particularly dynamic. Roughly half of all products experienced at least one price change during the study period, and price reductions were frequently followed by measurable increases in sales.

These findings resemble patterns commonly observed in competitive retail markets, where consumers compare products based largely on value and potency. In this environment, producers have strong commercial incentives to offer products that are both inexpensive and highly potent in order to attract patients.

Medical and Recreational Markets May Be Converging

The authors suggest that Germany’s medical cannabis market increasingly resembles what economists would expect in a conventional consumer marketplace rather than a strictly therapeutic medical system. Traditional medical prescribing generally prioritizes clinical evidence, individualized treatment goals, dosing precision, and risk management. In contrast, the observed purchasing patterns indicate that many consumers gravitate toward products that maximize THC while minimizing cost.

This does not necessarily mean patients are misusing medical cannabis. Many individuals may have legitimate medical reasons for preferring stronger products or seeking affordable treatment. However, the overall market trends suggest that commercial competition, not clinical evidence, is becoming an increasingly powerful force shaping which products succeed.

As a result, Germany’s medical market increasingly appears to function as a quasi-adult-use channel, with consumer demand favoring the cheapest and most potent products rather than evidence-backed formulations or lower-risk therapeutic options.

Does Higher THC Mean Better Medicine?

One of the central questions raised by the study is whether higher THC concentrations actually provide superior therapeutic outcomes.

Current scientific evidence does not consistently support the assumption that higher-potency cannabis produces better medical results across most conditions. While some patients may require stronger THC formulations for certain symptoms, many medical cannabis treatments can be effective at lower THC doses, balanced THC-to-CBD ratios, or carefully titrated dosing regimens. Increasing THC concentration may also increase the likelihood of adverse effects such as dizziness, sedation, anxiety, cognitive impairment, or intoxication, particularly in new or medically vulnerable patients.

The researchers argue that the commercial popularity of high-THC flower appears to outpace the available clinical evidence supporting its widespread therapeutic superiority, creating a challenge for policymakers attempting to balance patient access with evidence-based medicine.

Public Health Questions Continue to Grow

Germany’s medical cannabis reforms have undeniably improved patient access. Removing cannabis from the narcotics list simplified prescribing, expanded product availability, and helped reduce prices that previously limited treatment for many patients. And those are meaningful achievements. However, easier access alone does not determine whether a medical market aligns with broader public health objectives.

The researchers suggest regulators may eventually need to consider whether current market incentives encourage product development based primarily on consumer demand rather than therapeutic value. That does not necessarily mean restricting access to higher-THC products. Instead, it may involve encouraging more evidence-based prescribing, improving patient education, expanding research into optimal cannabinoid formulations, and ensuring that commercial competition does not overshadow clinical decision-making.

As medical cannabis continues to evolve internationally, Germany offers an important case study for other countries considering similar reforms.

Germany’s experience demonstrates how quickly medical cannabis markets can change once regulatory barriers are reduced. Greater affordability and broader product selection have made treatment more accessible for many patients. The study highlights an important policy question that extends well beyond Germany. If medical cannabis systems increasingly resemble retail marketplaces, regulators must determine how to balance patient choice, commercial competition, and evidence-based medicine.

Ultimately, the research suggests that Germany’s medical cannabis market is not simply growing; it is changing in ways that challenge the traditional distinction between medical and adult-use cannabis. Whether that evolution ultimately benefits patients or creates new public health concerns will likely depend on how future regulations adapt to an increasingly commercial marketplace.
