Cannabis Topicals: How to Use Them for Pain Relief

Cannabis creams, balms and lotions are marketed for localized pain, but their effectiveness depends on the condition and formulation. Small human trials have produced mixed results. A topical is not a proven treatment simply because it contains CBD or THC, and an ordinary balm should not be assumed to deliver cannabinoids into a painful joint or deep muscle.

How Cannabis Topicals Might Affect Pain

Cannabinoid-related signaling in the skin and peripheral nerves provides a rationale for research. However, a plausible mechanism does not show that a finished cream delivers enough of an ingredient to the relevant tissue or improves pain in patients. Formulation matters. A product intended to act locally differs from a transdermal formulation designed to move ingredients through the skin into circulation. A patch shape alone does not establish delivery, duration or effective pain relief. Neither type should automatically be described as free of systemic effects.

What Human Trials Have Found

  • Thumb-base arthritis: a 2022 randomized crossover trial involved 18 participants using CBD in shea butter or shea butter alone for two weeks per treatment, separated by a washout. Pain and disability improved with the CBD formulation, and no adverse events were reported. The small, short trial does not establish benefit for all arthritis, every commercial cream or long-term use.
  • Chemotherapy-induced neuropathy: a 2024 pilot trial of 40 patients compared CBD isolate cream with placebo, with a crossover after two weeks. Neuropathy scores were similar, and the study did not support the tested cream for painful established chemotherapy-induced neuropathy.
  • Exercise soreness: a 2026 randomized pilot study found no significant difference in muscle-soreness scores or blood myoglobin between CBD gel and placebo. Although one strength measure favored CBD at some time points, the authors did not conclude that the gel accelerated overall recovery or reduced soreness.

These studies concern particular preparations and outcomes. They should not be combined into a claim that cannabis topicals treat muscle soreness, nerve pain, swelling and arthritis equally well. Nor should improvement after massage or use of a mixed-ingredient rub automatically be attributed to its cannabinoid content.

Choosing a Format Without Assuming a Benefit

Cream, lotion or gel May spread more easily over an area. Cannabinoid concentration and other active ingredients; texture does not identify which pain condition it treats.
Transdermal product Designed for delivery through the skin. Evidence for the specific delivery system, dose, wear time and potential systemic effects.

Look for a complete ingredient list and a batch-matched laboratory report. A cannabinoid potency panel alone is not a complete contaminant screen. The total CBD in a jar is not the amount delivered to tissue with each application.

In a 2022 analysis of 105 hemp-derived topical products, researchers found frequent CBD-label inaccuracies. THC was detected in 37 products, including four labeled THC-free. This historical sample does not describe every current product, but it shows why labels should not be treated as guarantees.

How to Use a Product More Carefully

  • Follow the exact label for amount, frequency, application site and whether skin must be intact. Do not apply an oral oil or vape liquid as though it were a tested skin product.
  • Keep it away from eyes and mucous membranes. Wash hands after use unless the hands are the treatment site and the label provides different instructions.
  • Stop for a new rash, significant irritation or worsening symptoms. A small-area test may reveal irritation but cannot rule out every reaction.
  • Do not add heat or tight coverings to increase absorption. Products containing menthol, methyl salicylate or capsaicin can cause burns; follow their warnings carefully.
  • Store securely, in the original container, away from children and pets.

FDA safety information on topical pain relievers documents serious skin burns with certain counterirritant products and heating-pad warnings. This concern relates to those ingredients and formulations; a warming or cooling sensation is not proof that CBD is working.

When Pain Needs Assessment

Seek medical advice for persistent, worsening or severe pain, new weakness or numbness, major swelling, or a hot, red joint with fever. New symptoms during or after cancer treatment should be discussed with the oncology team. Do not use a topical to delay evaluation or replace a prescribed treatment.

For an established diagnosis, ask about appropriate exercise, physical therapy and other treatments with evidence for that condition. If trying a topical, track a meaningful outcome such as walking, hand function or sleep disruption, as well as adverse effects. Reconsider it if there is no useful improvement rather than applying progressively more.
