Medical Cannabis Benefits May Go Beyond Pain Scores

When people think about medical cannabis and chronic pain, the conversation often centers on a single question: does it reduce pain? And while that question matters, it may not tell the whole story. For many patients living with chronic pain, success is not necessarily defined by eliminating pain altogether. It may be the ability to get out of bed more easily, spend time with family, return to hobbies, complete household tasks, or simply enjoy life again. These improvements can be difficult to capture with traditional pain measurements, yet they often represent the outcomes that matter most to patients.

A large real-world study1 from the Minnesota Medical Cannabis Program is helping researchers better understand this distinction. The findings suggest that medical cannabis may be delivering benefits that extend beyond pain intensity alone, offering valuable insight into how patients experience meaningful improvements in their daily lives. Let’s dive into the study and what it means for the future of pain management research.

Inside the Minnesota Study

Researchers analyzed data from 6,010 patients enrolled in the Minnesota Medical Cannabis Program. All participants qualified for the program due to chronic or intractable pain and continued purchasing medical cannabis for at least eight months.

Unlike many clinical trials that evaluate a single cannabis product under controlled conditions, this study captured how patients actually use medical cannabis in the real world. Participants purchased a variety of products, including flower, vaporized products, edibles, oral solutions, concentrates, and products containing different THC-to-CBD ratios.

Before each purchase, patients completed a Patient Self-Evaluation that included the PEG scale, a validated assessment tool that measures pain intensity, interference with enjoyment of life, and interference with general activity. Researchers defined success as at least a 30% improvement in a PEG score within four months of beginning medical cannabis treatment, which is considered clinically meaningful because it reflects a noticeable change in a patient’s daily experience rather than a minor statistical shift.

The Numbers Tell an Interesting Story

The results revealed that many patients experienced meaningful benefits after starting medical cannabis. Among patients who entered the program with moderate-to-severe symptoms:

  • 54.9% achieved at least a 30% improvement in life enjoyment
  • 54.7% achieved at least a 30% improvement in general activity
  • 40.8% achieved at least a 30% improvement in pain intensity

The findings are notable because improvements in quality-of-life measures exceeded improvements in pain scores themselves. In other words, patients were more likely to report that they could enjoy life more and function better than they were to report substantial reductions in pain intensity.

Researchers also evaluated whether improvements persisted over time, and a majority of patients who achieved meaningful benefits during the first four months continued to report those improvements over an additional four-month period. The inclusion of this analysis highlights an important question in medical cannabis research, as it is not simply whether benefits occur, but whether they last. Understanding the durability of symptom improvement will be critical as researchers continue to evaluate the role of medical cannabis in long-term pain management.

Daily Function May Matter Most

The results highlight an important shift occurring throughout pain medicine. Historically, treatments have often been judged primarily by their ability to lower pain scores. However, patients frequently describe success differently. A patient may still experience chronic pain while being able to walk farther, sleep more comfortably, spend time with family, or participate in activities that were previously impossible. Those gains may not always translate into dramatic changes on a pain scale, but they can dramatically improve quality of life.

The Minnesota data findings demonstrate that medical cannabis is helping many patients achieve these kinds of meaningful functional improvements, and for individuals living with chronic pain, that matters. The goal is not always to erase pain completely. Sometimes the goal is to reclaim the ability to live despite it.

What Cannabis Products Were Patients Using?

Researchers also examined purchasing patterns to understand which products patients preferred.

They found cannabis flower dominated purchases throughout the study period, making them the most commonly used form of medical cannabis among participants. Among these purchases, roughly 84% were classified as high-THC products, which were the most frequently purchased products across every major product category. On the other hand, balanced THC:CBD products and CBD-dominant products represented a much smaller portion of overall purchases.

At first glance, these purchasing patterns might suggest that high-THC products are driving outcomes. However, the researchers found a more nuanced picture.

Product Type Did Not Predict Success

To better understand purchasing behavior, researchers used cluster analysis to group patients into five distinct purchasing profiles based on the types of products they bought during their first four months in the program. The groups included patients who primarily purchased high-THC flower, high-THC flower and concentrates, high-THC flower and vaporized products, balanced cannabinoid products, and mixed product categories.

Researchers then evaluated whether any particular purchasing profile was associated with better outcomes. Surprisingly, once they adjusted for purchasing frequency, no purchasing profile was significantly associated with improved PEG outcomes. This finding suggests that product category alone may not explain why some patients experience meaningful improvements while others do not. Other factors, including dosing patterns, frequency of use, symptom type, cannabinoid content, terpene profiles, and individual patient characteristics, may play important roles in determining outcomes.

The results highlight how much researchers still have to learn about how medical cannabis is used in real-world settings and what factors contribute most to successful pain management.

Cannabis Patients Need Better Guidance

While the study did not directly examine patient education or dispensary guidance, the diversity of purchasing patterns raises important questions about how patients navigate treatment decisions. If thousands of patients are purchasing a wide range of products and researchers cannot identify a clear product category associated with superior outcomes, it is reasonable to ask whether many patients are largely learning through trial and error.

The unfortunate reality is that despite the rapid growth of medical cannabis programs, many patients still receive limited education regarding cannabinoids, terpenes, dosing strategies, administration methods, and product selection. The result is a system where many patients are self-medicating while trying to navigate an increasingly complex marketplace.

As a medical cannabis patient myself, I find this deeply concerning. Patients deserve access to evidence-based education that helps them make informed decisions. Instead, many spend months or years experimenting with products, investing significant time and money trying to discover what works for their unique needs. My own experience taught me that cannabinoid and terpene profiles matter tremendously. Certain cannabinoids can work better for specific symptoms, while some terpene combinations provide more relief than others. In my case, products rich in specific cannabinoids and terpene profiles often perform significantly better than products selected based solely on THC content. However, the troubling reality is that I only learned these lessons because I became a cannabinoid research analyst and spent years studying the science.

That should not be necessary.

Patients should not have to become experts in cannabinoid pharmacology to maximize the benefits of their medicine. There should be cannabinoid and terpene education available everywhere medical cannabis is discussed. Patients should have access to educational materials, healthcare guidance, and evidence-based resources that help them identify products more effectively and with greater confidence.

The Minnesota study found that 54.9% of patients achieved meaningful improvements in life enjoyment and 54.7% improved their ability to engage in daily activities. While those numbers are encouraging, imagine what those numbers could look like if patients were consistently given the tools and education needed to make informed decisions from the start.

Improving access is important but improving patient education and resources are equally important.

Final Thoughts

The Minnesota study adds valuable real-world evidence to the growing body of medical cannabis research, and additional layer of depth, particularly in areas related to quality of life and daily functioning. The study also highlights the complexity of cannabis therapy, as no single product category emerged as a clear winner, demonstrating that individualized treatment approaches may be more important than simply choosing flower, edibles, or vaporizers.

Future research will need to explore how dosing frequency, cannabinoid combinations, terpene profiles, administration methods, and patient education influence outcomes. Until then, one message from the Minnesota data is becoming increasingly clear: the value of medical cannabis may not be measured solely by how much pain disappears, but by how much life returns. And for many chronic pain patients, that is the outcome that matters most.
