Easing Alzheimer’s Agitation with Synthetic Cannabis: Providing the Calm without the Harm

Alzheimer’s disease brings an array of challenging symptoms. One that is significantly distressing and dangerous for patients and caregivers alike is agitation. Characterized by excessive pacing, verbal outbursts, and even physical aggression, agitation affects nearly 40% of people with Alzheimer’s and is difficult to manage, especially in severe cases. Traditional treatments, including antipsychotic medications, have been used to address agitation but come with serious side effects like delirium and seizures. Enter synthetic cannabis, specifically dronabinol, a synthetic form of THC, the psychoactive ingredient found in cannabis. This synthetic option is now emerging as an effective therapeutic to reduce agitation in patients with Alzheimer’s due to groundbreaking evidence from a recent clinical trial.

A Promising Clinical Trial

In an eight-year study1 led by the Johns Hopkins University School of Medicine and Tufts University School of Medicine, researchers examined the effects of dronabinol on Alzheimer’s patients suffering from severe agitation. The results were promising: dronabinol reduced agitation by an average of 30% over the course of three weeks without the harmful side effects typically associated with current antipsychotic treatments. This was a significant breakthrough in the management of Alzheimer’s, potentially offering relief not just to patients but also to their caregivers.

The study, which included 75 participants across five clinical sites, tested patients’ agitation levels using established scales such as the Pittsburgh Agitation Scale (PAS). Patients who received dronabinol experienced a reduction in agitation from an average PAS score of 9.68 to 7.26, compared to no significant change in those who received a placebo. Notably, dronabinol was well-tolerated by the patients, providing similar calming effects as antipsychotics but without inducing dangerous side effects like seizures and delirium.

Dr. Paul Rosenberg, one of the study’s principal investigators, emphasized the importance of this discovery: “Agitation is one of the most distressing symptoms of Alzheimer’s dementia, and we are pleased to make positive strides forward in the treatment of these patients.” Brent Forester, M.D., added that dronabinol could also help reduce healthcare costs and improve caregivers’ well-being by minimizing the burden of constant agitation.

Dronabinol: A Synthetic Alternative with Real Benefits

Dronabinol is a synthetic form of THC that was approved by the U.S. Food and Drug Administration (FDA) in 1985, originally to help patients with HIV/AIDS regain their appetite and, later, to treat nausea and vomiting associated with cancer chemotherapy. While it is a lab-created version of THC, it shares many of the therapeutic benefits associated with the natural compound.

However, dronabinol differs in that it is isolated and synthetically produced, which raises a few points for consideration. On one hand, it allows for more controlled dosages and has been thoroughly studied for safety, leading to FDA approval. On the other hand, the synthetic nature of the compound doesn’t provide the full spectrum of cannabinoids, terpenes, and other components present in natural cannabis that could offer additional therapeutic benefits—a concept known as the “entourage effect.”

The side effects of dronabinol are also worth noting. Though typically well-tolerated, some common side effects include vomiting, nausea, dizziness, and feeling unsteady. Furthermore, more serious and rare side effects have occurred, such as blurred vision, hallucinations, hearing loss, and seizures. In contrast, naturally derived THC has been shown through various studies to offer a wide range of benefits while eliciting fewer side effects in its natural form.

The Case for THC

THC, whether synthetic or natural, has a wide range of benefits that are becoming increasingly recognized through scientific research. Despite this, natural THC continues to face scrutiny, with regulations surrounding its use remaining rigid in many regions, while the FDA-approved synthetic version, dronabinol, is more readily prescribed.

  • Pain Relief: One of THC’s most well-known benefits is its ability to provide pain relief, making it an effective treatment for chronic pain conditions. This could be particularly beneficial for Alzheimer’s patients, as they often suffer from pain that can exacerbate their agitation.
  • Anti-Aging Effects on the Brain: Studies have shown that THC has neuroprotective properties, which can help delay or even reverse some of the aging effects on the brain. In fact, research indicates that THC may promote cellular health in the brain, which could have potential implications for Alzheimer’s treatment beyond just agitation management.
  • Cognitive Function Improvement: Contrary to the misconception that THC impairs cognitive function, research has discovered low doses of THC may improve cognitive abilities in older adults. By enhancing memory and cognition, THC could help combat some of the cognitive decline that accompanies Alzheimer’s and may even serve as a preventative treatment in the future.
  • Appetite Stimulation: Many Alzheimer’s patients suffer from a loss of appetite, which can lead to weight loss and malnutrition. THC, both in its synthetic and natural forms, has been shown to increase appetite, helping patients maintain a healthier diet and better overall health.

Why Synthetic Cannabis While Natural THC is Scrutinized?

The contrast between the acceptance of dronabinol and the scrutiny faced by natural THC is puzzling to many. Both compounds offer similar benefits, yet the natural form continues to face stricter regulation despite research highlighting its therapeutic potential. One possible reason is that synthetic versions are more predictable, easier to study, and can be standardized in a way that natural cannabis, with its wide variety of strains and cannabinoid profiles, cannot.

However, as research into cannabis grows, the conversation is shifting. Studies like the one conducted by Johns Hopkins and Tufts demonstrate the effectiveness of cannabis-derived compounds in treating complex conditions like Alzheimer’s agitation. These studies could pave the way for broader acceptance of natural THC as a viable treatment option for not just Alzheimer’s but a wide range of medical conditions.

Final Thoughts

The rising prevalence of Alzheimer’s disease underscores the urgent need for continued research into therapeutic solutions. Alzheimer’s is the most common neurodegenerative disease in the United States, currently affecting an estimated 6.7 million Americans aged 65 and older, a number that is expected to reach 13.8 million by 2060.

The findings of this clinical trial are promising, highlighting the potential for synthetic THC, in the form of dronabinol, to provide much-needed relief from the agitation associated with Alzheimer’s disease. While this is a significant step forward, it also raises questions about why natural THC continues to be met with resistance despite its many proven benefits. As more research unfolds, synthetic and natural cannabis could become key tools in the fight against neurodegenerative diseases, offering patients and caregivers relief from both the mental and physical tolls of conditions like Alzheimer’s.

For now, dronabinol offers a glimpse into how cannabis-based therapies can reshape the treatment landscape—providing the calm without the harm.
